* Click on "Document Upload Form".

* Select "My Account" under the Category.* Select the Verification's under the "Problem With" section.

* Write your message on "Your Message" body (hint in picture below)

* Now upload your documents(If you have Government ID card upload front and backside or if you have Passport Upload single info page side only and if you utility bill upload utility bill or bank statement for address verification)

* Please keep in mind no upload your Utility bill more than 6 month's old or 6 Month's old Bank Statement and no upload expired Photo Id Card or Passport.
